Reablement
Short-term support to help people regain independence after illness or hospital.
Reablement is focused, short-term care that helps people relearn daily skills and rebuild confidence after a hospital stay, illness, or injury. Rather than doing everything for someone, our carers support them to do as much as they can themselves – safely.
The goal is independence. Most reablement packages run for up to six weeks, with support gradually
reducing as the person regains their abilities and confidence at home.

How we deliver
Reablement is delivered in close partnership with NHS discharge teams and community health
professionals, with progress tracked and shared so everyone stays aligned on the person’s recovery.
